Carla and Elliot are arguing again. Turk and J.D watch on. Turk advises J.D to remain quiet. J.D can't help but comment on the bickering. They then turn their anger on him.
As he enters the hospital, J.D recounts something he was told when starting at the hospital, one out of every three patients who is admitted to the hospital dies there. That said three patients are being attended to by J.D and the other staff.
An old lady is admitted to the hospital first. She is very easy going and down to earth and she and J.D get along well. It's decided that she needs dialysis but she surprises J.D by saying that she doesn't want it. J.D tries to get her to reconsider but she tells him at 74 she's ready to die.
After being critacised by J.D about not taking time to understand and get to know his patients Turk takes time to chat to a patient whom he will be operating on. They have a lot in common and Turk spends more time with the patient. Whilst operating complications are found which requires more surgery.
Carla and Elliot are still finding it hard to get along. Elliot has to call someone to translate for a Spanish speaking patient who has been admitted. Carla is the translator who is appointed. Working together proves difficult and Carla enjoys watching Elliot struggle. Elliot has to make a decision which could determine whether the patient lives or dies. She can't make it, Carla realises she's struggling and helps her through it improving the relationship between the two.
The statistic mentioned by J.D at the beginning of the shown is called back into memory. All three of the patients being tended to by the staff at the hospital die, leaving J.D to muse about all three happening at the same time.
