It's nearly Christmas and a young man, Mr. Milligan, is brought into the hospital with his son Tyler. At first it seems that there is little wrong with him and J.D (Zach Braff) promises both Tyler and his dad that everything should go smoothly and they will be out of the hospital before Christmas. Dr. Cox (John C. McGinley) pulls J.D up on this promise and hints that statements like that will only get people into more trouble. As soon as Dr. Cox has finished the man slips into a coma suffering from something which they cannot diagnose easily. Thanks to interjection from Carla (Judy Reyes) the problem is found with the man, who would have been close to death otherwise. Quick surgery from Turk (Donald Faison) allows the man to go home before Christmas as J.D promised.

Elliot (Sarah Chalke) is charged with looking after Tyler. At first she is apprehensive as she doesn't feel she gets along well with children. She overcomes this quickly enough, however she looses Tyler as he heads of to look for chocolate milk.

J.D is in charge of some of the new pre med students. He's asked by on of them what his best moment as a doctor has been. This sparks a flashback scene of J.D when he first started at the hospital. After his session J.D asks the staff what their best moments are. They deflect the question but throughout the day when they have a spare moment each wonders back in time to their best moment. Elliot remembers helping a pony farmer who lent her one of his ponies, Turk thinks of the time when he bet the record for fastest appendectomy in the hospital and Carla remembers being thanked by one of the doctors.

Dr. Kelso (Ken Jenkins) hears that the Mr. Milligan does not have suitable medical insures and orders he be sent to the county hospital instead. J.D gets Turk to try and change Kelso's mind but he isn't successful. Dr. Kelso has a change of heart after Tyler wonders into his office and chats to him.

Turk is fretting as a new hospital policy allows family members of patients who are undergoing surgery to watch as it's carried out. Turk has to perform a few operations under the stress of on looking family. He allows Tyler to watch his dad being operated on though and gets thanks for doing so.
