The interns are looking toward the future and thinking about the next steps of their professional life now their interns are coming to a close. J.D (Zach Braff) and Turk (Donald Faison) are staying on as is Doug (Johnny Kastl) who has to retake his third year. Elliot (Sarah Chalke) is going to look into a specialist field of medicine. Carla (Judy Reyes) suggests Elliot tries for the position of chief of medicine at the hospital, a job which J.D feels is in the bag for him. After similar words from Molly (Heather Graham), Elliot calls out to Dr. Cox (John C. McGinley) who is happy to let her apply. When the results are announced Dr. Cox reveals Elliot is the new chief of medicine. J.D is shocked and snaps at Dr. Cox who then reveals J.D is also the chief of medicine, stating that between the pair they make one half decent doctor.
Carla gets more and more annoyed at Molly who is getting along well with most of Carla's friends. As Molly is a psychologist she soon finds herself helping out with the staffs various problems, a informal job which Carla took pride and comfort in having. As it becomes too much for Carla she attempts to subtly lash out at Molly. This doesn't work though and Carla is left to make friendship through Turk. The pair decides that they will both help out where needed.
For a paper Molly needs to submit she chooses to study the conflicts experienced between the staff in the hospital. She is assured by Dr. Cox that no such problems exist but it soon becomes clear that this isn't true. Cox is pleased with having a chance of giving some good news to a patient for a change. As her arrives at the patients bed he finds Turk has already told the news. Cox swears for revenge and the pair spend time trying to out perform each others. Elliot and J.D are also battling. The pair are tense due to their recent break up. The fact that they're both chief of medicine and have to work together fairly closely only makes matter worse. A man with a light bulb stuck inside him which no-one can work out how to remove and a patient crashing pull everyone together again.
