J.D starts the new year as an attending at Sacred Heart. He's keen to act as friendly as possible to his interns, especially as he remembers how intimidated he was by Dr. Cox in his first years as a medical student. He's hopes are laughed at by Dr. Cox and Dr. Kelso who tell him that all attendings come down hard on their interns. Eventually they're proved wrong as J.D realises his interns are just humouring him because he's "higher up the foodchain". J.D and Dr. Cox also meet as he goes behind J.D's back and gets his interns to work the system to help a patients chances of getting medical treatment. J.D is frustrated at first but he sees it from Dr. Cox's point of view and even covers for Dr. Cox when Dr. Kelso demands to know who's responable for the system manipulation.
Turk attempts to keep Carla on the pill after they increase their attempts to concieve and things begin to move to fast for him. He also allows J.D to move back into the flat without letting Carla know. Carla finds out about the pill after Turk resorts to drastic measures to get her to take them without knowing. Carla finds the pill hidden in a brownie Turk had given her.
Elliot struggles to fit in at the new hospital she's working in. In an attempt to stamp her authority in the hospital she begins telling everyone that she's a fantastic doctor and doesn't need there help. This lie soon catches up with her though. A few words from an angry Carla, who is annoyed at the antics of all three of her friends, sets Elliot on a clearer path though and she attempts to make new friends at the hospital.
